Yes. The rent for our community residence and apartment treatment program is set annually by social security at the congregate level II rate. The rent for our supportive housing program is based on 30% of total monthly income.
We do not have any clinical or medical professionals on staff. All of our residents receive these services from linkages within the community.
Unfortunately, we do not offer family housing at this time.
Unfortunately, we do not offer single apartments at this time. The majority of our Supervised Community Residence programs are shared bedrooms, while a select few are single bedrooms with shared living space. All our Apartment Treatment and Supportive Housing homes are shared housing with three adults, and each have their own bedroom.
The Way Back, Inc provides transitional, and permanent, supported housing programs but does not provide shelter housing options. You may contact your local Department ofSocial Services Office (DSS) for immediate shelter placement.
All applications for mental health housing, regardless ofagency, are submitted through Single Point of Access (SPA). A printable application and the contact information for SPA can be found at www.spahousingli.org.
The Way Back programs are designed to provide a continuum of services to our residents as they move toward independent living. The Agency provides rehabilitative housing services and staff support as a primary mechanism for achieving their goals. The primary emphasis of the program is to prepare residents to live independently in the community. A willingness to participate in the rehabilitative goal planning process is required.
The Way Back provides housing to residents of Suffolk County.
We serve individuals ages 18 and over who have a primary diagnosis of a serious mental illness (SMI). The Office ofMental Health’s definition of SMI is “Adults with serious mental illness are persons aged 18 or older who currently have, or at any time during the past year have had a diagnosable mental, behavioral or emotional disorder of sufficient duration to meet diagnostic criteria specified within the Diagnostic Statistical Manual (DSM). To be considered SMI, the diagnosable disorder must result in functional impairment that substantially interferes with or limits one or more major life activities.”
The Way Back offers three levels of housing: Supervised Program, Apartment Treatment Program and Supportive Housing Program.
• At the Supervised program, residents live in a community residence that are staffed by trained counselors who are available 24 hours a day for support and assistance. The emphasis of this program is to educate residents in acquiring basic living skills.
• At the Apartment Treatment program, residents live with two other roommates in a three-bedroom house owned and operated by the Agency. The counselors do not live on site, but they do make house visits up to five days a week and assistance is available 24 hours a day for emergencies. Residents assume more responsibilities for household activities, daily living skills, medication management, etc. in their preparation for independent living.
• At the Supportive Housing program, residents live with one or two other people in a house owned and operated by the Agency. Supportive Housing is permanent housing and residents pay their own rent to the Agency and case management services are provided to the residents. Supported Housing offers a sense of home so necessary to the enrichment of life.
The Way Back, Inc. can explore with you structured, purposeful activities in the community that are rehabilitative. The Way Back works with residents and their service providers to assess the type of activity that would be beneficial for their recovery.
