Home and Community Based Services

Home and Community Based Services provide opportunities for Medicaid beneficiaries to receive services in their own home or community rather than institutions or other isolated settings. 

These individualized, focused services are designed to assist people in recovery with mental health and/or addiction challenges to achieve their goals — leading to greater independence and improved quality of life. 

In order to receive HCBS services, you must be an adult Medicaid beneficiary, age 21 and over, and enrolled in a HARP managed care plan through the New York State Department of Health.  Please contact us to explore your eligibility.
